IMPORTANT:

  • Your personal data will be transferred by means of this form over the internet in a secure way.
  • You will need to bring your original CNIC and the supplementary applicants original CNIC when you come to collect your Supplementary Credit Card from your preferred SCB branch.
  • Approval of the facility is subject to credit checks as per the Bank's policies and regulatory requirements.
  • Maximum of 5 Supplementary Cards can be obtained against one basic card.
  • This Supplementary Card can only be issued to dependents of the primary card holder (i.e. family members where relationship can be established via the CNIC of primary credit card holder).
  • You will be notified on your mobile number, when cards are ready for collection from the selected branch.

Supplementary Cards can be issued to immediate family members who are Pakistani nationals and have valid CNICs / SNICs.
All fields in this form are mandatory. The Supplementary Card will be delivered to your preferred SCB branch.
